Improved Monthly Contribution Report easier to fill out & send

HOOPP has developed a new, “fillable” version of the Monthly Contribution Report that is now available.
Please let the person responsible for filling out this form – which reports monthly HOOPP contributions – know about the new form, which is both easier for that person to complete, and now can be remitted by e-mail.
The form is fillable, which means that when a user opens it up on his or her computer screen, information can be typed directly onto it using a computer keyboard. Once the user has finished, the form can be sent to us by e-mail by clicking the "E-Mail" button at the top of the form.
The Monthly Contribution Report will be accepted even if there is no signature, and thus is the only HOOPP form that can be submitted by e-mail.
Advantages of this form include:
- ease of use for your team – no handwriting required, form can be e-mailed
- easier to enter data on the form, as it can be enlarged on the computer screen; areas that need to be filled out are highlighted
- users can make a copy by hitting the "Print" button
- form can be e-mailed by hitting the "E-mail" button
- data will be easier to process by HOOPP, again because we will receive it more quickly, and there will be no legibility issues
Employers who prefer printing the form out and filling it in by hand are still able to do so.
The Monthly Contribution Report is the first HOOPP form to be made fillable. We plan to make our other forms fillable in the near future. Fillable PDF forms are a first step towards a future state in which employers will be able to log in and remit data online.
