Enrolment Form discontinued; MDC CDs coming soon

Enrolment Form To Be Discontinued

Effective Oct. 1, 2009, the enrolment process will be handled exclusively via HOOPP ESE with the paper Enrolment Form as well as the Re-enrolment Form For Retired Members being discontinued.

In the rare instance that you should come across a special case in which the enrolment cannot be processed on HOOPP ESE, you may complete the Special Circumstances Enrolment form, which can be accessed through this list of forms available on hoopp.com.

The three-step enrolment process on HOOPP ESE is a faster and more streamlined way of enrolling new members into the Plan and will makes things easier for you.

Please Prepare For MDC CDs

It's that time of year again: the 2009 MDC Reporter package will arrive in your office by early December, but to help ensure your 2009 MDC Reporter CD is up to date, you can request reports showing termination, enrolment and non-health leave data processed in 2009 by e-mailing mdchelp@hoopp.com.

The data will be provided to you on a password-protected spreadsheet, using your employer code as the password.

Once you have reviewed the reports you should process any outstanding termination and enrolment information through HOOPP ESE by Nov. 20 so that your 2009 MDC Reporter CD will be properly updated.

Reminder: Non-Health Leaves and HOOPP ESE

If you have any members who are on non-health leaves, please process these on HOOPP ESE rather than MDC.

Example: David was on a personal leave from January 2009 until March 2009. Because this leave happened within the 2009 calendar year, it can be reported through HOOPP ESE.

If, however, the member is on a non-health leave that crosses over a calendar year, please send this information to HOOPP on a password-protected spreadsheet using your employer code as the password.

Example: Susan, however, starts a personal leave in November 2009 and won't be back to work until February 2010. Because this leave starts in 2009 and crosses over into 2010, it should be sent to HOOPP via spreadsheet.
