The 2009 Data Request for Pension Adjustments is due Jan. 11 – if you have not already sent it in, please note that it's vital that we receive this information so that we can provide you with the PA information required for the member's T4.
The information contained in the PA report is used by HOOPP to calculate free accrual for members who were on a health leave in 2009. To receive free accrual, the member:
- had to have been on a health leave for more than 15 weeks in 2009
- must have submitted medical evidence of his/her disability that has been approved by HOOPP's physician
HOOPP also uses the information on the report to provide employers with the necessary pension adjustment for reporting on the member's T4.
Every effort will be made to provide you with the necessary PA information by the end of February. In the event that you will be issuing your T4s before the end of February, we suggest that you withhold issuing the T4 for any member who you included on your report so that a duplicate T4 will not need to be issued.

Please use this calculator to get a member's PA based on annualized earnings and contributory service. A PA is required for tax reporting purposes.
