Fillable Forms
Due to popular demand, HOOPP now provides “fillable” forms on hoopp.com.
The updated forms have exactly the same content, but now allow for users to type directly onto the computer screen before they are printed off, signed, dated and mailed to HOOPP.
This makes the forms easier to fill out and easier for HOOPP to read the information that is sent to us.
Note, you can still print the forms out and fill them in manually if that’s your preference.
The fillable forms are a first, small step towards other planned process improvements for members and employers, including web-based services. HOOPP is working on other ways to streamline our processes and will keep you in the loop as new developments occur.

All the forms on hoopp.com have already been updated. To access the new versions, just click on the “Download PDF” buttons as you would before.
