Revised Health Leave Form Show How We Listen To You
At HOOPP, we’re always open to suggestions from employers because it’s important to us that we help make your job a little bit easier.
Several employers voiced concerns about the Health Leave form and suggested making certain changes.
Betty Kenney, Benefits Program Administrator at St. Joseph’s Healthcare, one of the administrators who contacted us, says, “In our view, the old LOA form was time consuming and cumbersome, as each leave situation differed and work accommodation programs required obtaining a detailed breakdown of the earnings information when on reduced hours, which can vary week to week.”
She suggested shifting certain questions to allow the employer to skip to the next section if certain answers didn’t require any further information.
We took what Betty had to say to heart and consulted with our doctors on staff to help streamline the new Health Leave form so it would be easier to fill out.
After reviewing the new form, Betty says, “We thank HOOPP for listening and responding with this new streamlined Health Leave Form. We believe this should ease some of the burden for the province’s operation managers, human resource and occupational health practitioners.”
