Tips on How Best to Complete HOOPP Forms
We've had a few questions and comments from employers in recent months, asking about how to find our forms and how to fill them out correctly.
To help you, here's some information on how to find and fill out the various HOOPP forms. Remember as well that details on how to fill out forms can be found in the online HOOPP Administration Manual.
Finding forms: A complete list of all the HOOPP forms can be found here.
Fillable forms: To ensure forms are easily read, we recommend that you take advantage of their “fillable” feature. Last year, in response to employer suggestions, all of our forms were upgraded so that you can type information directly on sceen before you print them out.
Include SIN: Always include the member's social insurance number. While HOOPP is planning to move away from the use of SINs on member records, they are for now the main identification number for our membership.
Paper size: Please print bar-coded forms on the correct size of paper. Legal sized forms should be printed on legal sized paper (8 ½" by 14") and letter sized forms on letter-sized paper (8 ½" by 11").
Easier to scan when mailed: All forms that are sent in to HOOPP are scanned and therefore, should be mailed in instead of faxed so that the forms will be clear and legible.

Work is well underway on providing a way for employers to remit information online – an alternative to having to print out forms and send them to HOOPP.
Don't send original documents: Proof of age documents should be photocopied – never send in the originals and never fax these documents as they are rarely legible.
Please note that the following are acceptable proofs of age:
- valid Canadian passport
- birth certificate
- baptismal certificate
- citizenship papers
- valid Canadian driver's license
Please note that only the following types of Power of Attorney are allowed to make changes on behalf of members and pensioners:
- general power of attorney
- property power of attorney
Notarized or certified proof of this authority must accompany the completed forms before HOOPP can process any changes.
