Retirement Forms for Employers

Name	Purpose
Retirement Notice (legal sized) French	Members and employers jointly complete this form to apply for a HOOPP pension. (Complete at least three months before retirement.)
Service, Earnings, & Contributions Report (legal sized)	An employer completes this form to report final service, earnings, and contribution information for retiring member.
Employer Request for Pension Estimates French	An employer completes this form to request pension estimates from HOOPP. The form must be signed by the member for whom the estimate is being requested.
Pension Resumption Form (legal sized) French	A retired member who has temporarily stopped his or her pension to re-enrol in the Plan uses this form to resume receiving pension payments (and to stop making HOOPP contributions).

Please print bar-coded forms on the correct size of paper. Legal sized forms should be printed on legal (8½" x 14") paper, and letter sized forms on letter (8½" x 11") paper. Also, please send bar-coded forms to HOOPP by mail. Bar codes can't be scanned correctly if they are printed on the wrong size of paper or are faxed.
