Proof of Qualifying Spouse
Married
HOOPP will accept a copy of your marriage certificate as proof of marriage.
If you can’t obtain a copy of your marriage certificate, HOOPP will accept a statutory declaration indicating the date you got married and your spouse’s name.
Common law
If you are in a common-law relationship, HOOPP will need a statutory declaration that your partner meets HOOPP’s definition of a qualifying spouse.
A statutory declaration is a sworn statement made before a judge, notary public, or commissioner of oaths.
