30 Days’ Notice to Change to Your HOOPP Record

Please be aware that HOOPP requires 30 days from receiving your Member Information Change Form (which you can obtain from your employer or download) to change:

• your name
• your address

The same 30 days’ notice applies if you wish to add or delete a spouse from your record, or change your beneficiary. To do any of these things, please use a Beneficiary Designation Form, also available from your employer.
