Help Make Document Processing Easier

To prevent processing delays, HOOPP asks that you remember to provide your social insurance number on every form.

Also please mail forms in as opposed to faxing.

All forms that are sent in are scanned at HOOPP. When copies of proof of age documents such as driver’s licences are faxed, they are rarely legible. To avoid the inconvenience of a delay in processing your benefits, please mail in photocopies of proof of age documents – never the originals.

To further ensure that nothing prevents forms from being scanned correctly, please print bar-coded forms on the correct size of paper. Legal sized forms should be printed on legal sized paper (8 ½” by 14”) and letter sized forms on letter-sized paper (8 ½” by 11”).

To make it easier for HOOPP to be able to read the information on forms, try filling them out onscreen. You can now fill out the forms by typing directly onto your computer screen before printing.
