Provide and Double-check Your SIN When Mailing or Faxing

When you are corresponding with HOOPP via fax or mail, please remember to include your social insurance number (SIN). HOOPP needs your SIN on all documentation submitted for verification purposes in order to process any requests. If you take an extra minute to confirm that you are providing the correct SIN, you will avoid any confusion about member identity and any resulting delays in service.

NOTE: Please do not include your SIN in e-mail messages as security can not yet be guaranteed. HOOPP is working towards providing members with unique identifiers to protect their privacy and deliver a secure means of electronic communication.

Did you know you can also fill out HOOPP forms electronically?

The updated forms have exactly the same content but now allow for users to type in their information before the forms are printed off, signed, dated and mailed to HOOPP. This makes the forms easier to fill out and easier for HOOPP to read. Of course you can still print the forms off and fill them in manually if you prefer. Click here to access forms for pensioners.
