Spousal Info Needed For Estimates
With revisions made to both the Employer Request for Pension Estimates and Member Request for Pension Estimates Form in August 2007, room was added for spousal information, with the assumption that, if the space was left blank, then the member was indicating that he or she was single.
If a member does not have a spouse on file with HOOPP, it is vital to either direct members to these new forms or to remind them that, if they're still using the outdated forms, the estimates will assume that the member is single.
