HOOPP's forms are now fillable onscreen
The updated forms have the same content, but now you can fill them out directly on your computer screen, then print and mail to HOOPP. If you prefer, you can still print the forms and fill them out manually.
HOOPP's forms use the PDF format. For help, read Help Using PDFs and Adobe Reader.
Sample letters & waivers
These templates, in plain text, are to be reproduced on your organization's letterhead. Cut and paste the text into your word processing program, then print it. Make copies for you and, if applicable, the member.

Instructions
Most forms are formatted for letter-sized paper (8.5" x 11"). Some are formatted for legal-sized paper (8.5" x 14"). If you are submitting a legal-sized form that has a bar code, please print it out on legal paper -- this allows us to scan the form and bar code correctly.
These forms should be sent by mail, and not faxed, so bar codes can be read. Please do not fold forms as this impairs the scanning process.
