Easier to Fill HOOPP Forms

In response to suggestions from members and employers HOOPP is now providing fillable versions of all forms.

You can use HOOPP forms to:

• update your beneficiary designations
• add or remove a spouse
• give HOOPP your new home address

The updated versions have the same content as the original forms, but now you can fill out the form on your computer before you print it off.

This makes things easier to read and avoids having to fill out forms by hand.

You can still print the forms and fill them out manually, if that’s your preference.
