HOOPP offers forms and links to third-party websites for pensioners. Below are descriptions and details of the resources.
HOOPP forms

| Pensioner Information Change Form | Use this HOOPP form to report a change in your name, address or banking. Do not use this form for a change in beneficiaries, instead use the Pensioner Beneficiary Designation Form. |
| Re-enrolment Estimate Request Form | Use this HOOPP form if you receive a HOOPP pension, but also work (or plan to work) for a HOOPP employer, to find out how much additional pension you'll get if you temporarily stop your HOOPP pension and resume contributions. |
| Post-retirement
Spousal Benefit Application (Legal sized*) |
This HOOPP form can be used by a pensioner who has a new spouse after retirement and wants to provide spousal benefits. Conditions apply. |
| Pensioner Beneficiary Designation Form | Use this HOOPP form to change non-spouse beneficiaries. The spouse you had at retirement is by law first in line for benefits payable upon your death unless a waiver was signed and received by HOOPP prior to the commencement of your pension. If that spouse has died, you can name a new spouse by using a Post-Retirement Spousal Benefits Application. Certain conditions apply. |

TD1 forms - on Canada Revenue Agency's website (Residents of provinces and territories except Quebec) |
The TD1 forms allow you to manage the taxes deducted from your pension. There are both federal and provincial/territorial forms. Complete only the federal TD1 form to have additional tax deducted from your pension. Complete both the federal TD1 form and the TD1 for your province/territory if you have tax credits that entitle you to reduce the tax withheld from your pension. (See our Taxes page for info) |

*Legal sized forms are formatted to fit on 8.5 x 14 inch paper. However, it will print on either legal or letter sized paper. We prefer you use legal sized paper, but either size is acceptable.
